When filling out the registration form, please ensure you fill in all blanks. If something is not applicable, write N/A or cross a line through it.
Emergency contacts have to have a legal land address, not a box number or a rural route please. These are licensing requirements. Little Star does work with subsidized families, for more information on subsidy please navigate to our resources page.
If you have any questions or concerns, please contact Miss Becki through our contact form. Completed forms and admin fees are to be dropped into the drop slot beside the front door at the 5009 Broadway Ave location and an email confirmation will be sent out once processed.
First come, first registered as we will process forms as they come in.
Please do not pay the admin fee until you receive your email confirmation. Admin fees are paid by cash or can be e-transferred to email@example.com. Little Star is happy to announce we now have automatic deposit for e-transfers so no questions/answers are needed, please include your child's name and description of what you are paying for in the comments.
We require a hard copy of the registration form which can be dropped in the drop slot located beside the front door of our 5009 Broadway Ave location.
